Mini-Workshops Instructor Application Form

Thank you for your interest in becoming an Instructor for our Marvelous Mini-Workshops. A separate application is required for each different course you wish to offer.

To start the application, click on the Next button below. You can save your progress and return to the form later by clicking on “Save and Continue Later”; you will be sent an email with a link that will return you to the place you left off. NOTE: you must be logged-in to the members-only website in order for the Continue link to work; otherwise, all you will get is the login page.

You will be asked to provide a list of tools and materials that 1) you will provide (either free or for a fee of no more than $15) or loan, and 2) students will be required to bring to class themselves (at a reasonable cost to the student), as well as any optional items that may enhance their experience.

Supplied Materials

We need to know what tools and materials will be required for this class in order for a student to be successful, as well as any optional items that may enhance a student’s experience. Don’t forget to list all the items required or you may have to supply them yourself out of your own pocket.

Please indicate whether or not you will be providing at least some, if not all, materials to your students, including if you will be loaning students the use of some of your tools for the class.
Provide a detailed list of the items you will supply to students. (Not just “complete kit” or similar – we need a breakdown.) If you will be loaning tools to students during the class, list them here and mark them as “(loan)” Please enter as a bulleted list starting with a dash and space, and capitalize the first letter of each line.
How much will you be charging for the materials you will be providing (as described in the previous question)? The amount of the fee must reflect the cost of materials.

If you expect that your students are likely to already possess all/most of the required materials and tools listed above, and wish to make the materials fee optional, indicate so here.

If students will be required to bring essential tools or materials to the class, please indicate so here.
Please indicate what materials, tools, or other items that a student MUST bring to class in order to succeed. Do not include ‘nice to have’ items, as this is covered separately below.
Please provide your best estimate (a range is OK) of the cost of the student-supplied items, assuming they must purchase them for the class. Do not include common household items (scissors, for example).
List any materials or tools that would enhance a student’s experience at the workshop., especially items that they may already own. For example, fancy scrap-booking scissors, paper cutters, staplers, hot glue guns, etc. These items cannot be REQUIRED for the student to enjoy the class.
